OATA Meeting Registration

General info:  Please complete all information below.  At the conclusion of this form, the two payments options will be explained including a link to the PayPal® site.  If choosing to pay using this option, please be sure to submit the registration form by clicking on the “submit form” button.  Please register on or before May 1^st; after May 1^st there is a $25.00 late fee.    A confirmation email will be sent to after receiving your registration. 

Refund Policy
Late registration for the clinical symposium is May 1^st, 2010.  In an event you are not able to attend the meeting after registering for the meeting a full refund will be given if the meeting.  To obtain a refund, you must place a written request on or before May 1^st, 2010 to newman.dan@uninonps.org to receive the refund.  If the cancellation comes after May 1^st, 2010 a 50% refund will be given to the participant.  Please contact the meeting committee chair with any further questions.

May 21st, 2010-  The Fifth Annual Golf Tournament & “19^th Hole Social"
This year’s tournament will be held at the the Canyons at BlackJack Ridge (http://canyonsatblackjackridge.com),  located in Sand Springs.  The cost of this year’s event will be $50.00 which will include cart, green fees, and the 19^th hole social.  This year's social will take place following the tournament, location TBA.  A shotgun start will begin at 1:30 pm.   Foursomes are encouraged, if you do not have a foursome you will be placed in one at the course.  If planning to attend the golf outing and have a specific foursome you would like to play in, please forward the players in your group to newman.dan@unionps.org.

Very easy to use, I would use it again. Adequate to use, but I prefer paper mailing. Difficult to use, but would use if improved. Extremely difficult to use, I will mail my check next time.
