OATA Meeting Registration

General info:  Please complete all information below.  At the conclusion of this form, the two payments options will be explained including a link to the PayPal® site.  If choosing to pay using this option, please be sure to submit the registration form by clicking on the “submit form” button.  Please register on or before May 1^st; after May 1^st there is a $25.00 late fee.    A confirmation email will be sent to after receiving your registration. 

Refund Policy
Late registration for the clinical symposium is May 1^st, 2012.  In an event you are not able to attend the meeting after registering for the meeting a full refund will be given if the meeting.  To obtain a refund, you must place a written request on or before May 1^st, 2011 to newman.dan@uninonps.org to receive the refund.  If the cancellation comes after May 1^st, 2012 a 50% refund will be given to the participant.  Please contact the meeting committee chair with any further questions.

May 19th, 2012-  The Seventh Annual Golf Tournament & “19^th Hole Social"
Plans are being made for this year's annual golf outing.  Please check back for more information.

Very easy to use, I would use it again. Adequate to use, but I prefer paper mailing. Difficult to use, but would use if improved. Extremely difficult to use, I will mail my check next time.
